29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  On or about March 12, 2015, employees were exposed to damage to the eyes when the employees used materials such as, but not limited to, Toyocoat, n-methylpyrrolidinon, and sulfuric acid.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

On or about March 12, 2015, employees were exposed to injury to the hands when the employees used materials such as, but not limited to, Toyocoat and n-methylpyrrolidinon and sulfuric acid.  a.  employee non-recordable injury due to Toyocoat and n-methylpyrrolidinon on March 7, 2015 b.  employee non-recordable injury due to sulfuric acid on January 7, 2015   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
