29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):            (a):   On or about March 9, 2015, at the above addressed worksite, located at the east side of the building, employees were exposed to the hazard of falling a distance of 9 feet 6 inches to ground level while working from a single plank on a fabricated metal frame scaffold without a conventional fall protection system.            (b):   On or about March 6, 2015, at the above addressed worksite, located at the south side of the building, employees were exposed to the hazard of falling a distance of 9 feet 6 inches to ground level while working from two planks on a fabricated metal frame scaffold without a conventional fall protection system.

(b):  On or about March 6, 2015, at the above addressed worksite, located at the south side of the building, employees were exposed to the hazard of falling a distance of 9 feet 6 inches to ground level while working from a two plank fabricated metal frame scaffold that was missing base plates.

29 CFR 1926.405(a)(2)(ii)(I): Protection was not provided to avoid damage to flexible cords and cables used for temporary wiring which passed through doorways or other pinch points:              (a):   On or about March 9, 2015, at the above addressed worksite, employees were exposed to electrical hazards while using portable power tools such as a hammer drill and a circular saw plugged into extension cords laying on the ground that passed through a doorway and then around the corner of the building and up the frame of a fabricated metal scaffold.
