29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or another provision in paragraph (b) of this section which provides for an alternative fall protection measure: (a)Alliance Roofing Inc, at 1246 S Race Street, Denver, CO: On and before 3/13/15, Alliance Roofing Inc did not ensure that each employee engaged in residential construction activities 6 feet or more above lower levels was protected by guardrail systems, safety net systems, personal fall arrest systems, or another provision in paragraph (b) of this section which provides for an alternative fall protection measure. Two employees were setting ridge caps and cleaning up following roofing of a two story home. One employee, while cleaning debris from the second story roof top, was not protected by a guardrail system, safety net, or personal fall arrest system. This condition exposed the employee to an approximate 19.5' fall hazard.

Two employees were setting ridge caps and cleaning up following roofing of a two story home. While completing the roofing work, the employees utilized personal fall arrest systems. The lanyards of each worker were attached to each rope grab device at the end of each fifty-foot lifeline. The PFAS's were not rigged to prevent a free fall of more than 6 feet. This condition exposed the employees to an approximate 19.5' fall hazard.

Two employees were setting ridge caps and cleaning up following roofing of a two story home. While completing the roofing work, the employees utilized personal fall arrest systems. 29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a)Alliance Roofing Inc, at 1246 S Race Street, Denver, CO: On and before 3/13/15, Alliance Roofing Inc did not ensure that portable ladders that were used for access to an upper landing surface did not have the ladder side rails extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access. Two employees were setting ridge caps and cleaning up following roofing of a two story home. The workers accessed the first story roof via an aluminum extension ladder. The side rails of the extension ladder extended one rung past the roof edge. This condition exposed the employees to an approximate 10.2' fall hazard.
