29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access: a) Jobsite, 120 to 124 Race Street - The portable ladder used to access the third floor did not extend at least three (3) feet above the upper working surface exposing employees to a fall hazard during access and/or egress. Employees were performing framing operations on the third floor, approximately 21 feet above ground level. Observed on or about 3/13/15.
