29 CFR 1910.23(a)(3): Every hatchway and chute floor opening shall be guarded by one of the methods referenced in sections (i) and (ii): a) On or about March 13, 2015, and at times prior, employees were exposed to fall hazards when they worked near the second floor tire hatchway/opening used to drop tires from the second floor storage area to the shop on the first floor. This floor opening was greater than 10 feet above the first floor and was not guarded with standard railings.
