29 CFR 1910.178(q)(7): Industrial trucks were being placed in service after the examination showed condition(s) adversely affecting the safety of the vehicle. On 13 March 2015, and at times prior thereto, a Mitsubishi Diesel Fork Lift, Model FD55N, had a damaged seat belt in which the belt would not retract and had been pulled out to the extent of the length of the belt, exposing employees using the fork lift to a struck by and crushing hazard in the event of the lift abruptly stopping or flipping over.

29 CFR 1910.253(b)(2)(iv): Valve protection caps, where cylinders were designed to accept caps, were not always in place, hand-tight, except when cylinders were in use or connected for use. On 13 March 2015, and at times prior thereto, two cylinders in storage were missing the valve protection caps, exposing employees nearby in the yard and on the other side of the corrugated metal wall of the building to the hazard of being struck by either cylinder in the event of damage to the valve stem resulting in the cylinder becoming a projectile.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour. On 13 March 2015, and at times prior thereto, oxidizers and flammables were stored within the same cylinder cage along the east outside wall of Bay 3, exposing the employees working nearby within the yard or adjacent to the wall within Bay 3 to an explosion or fire hazard.

29 CFR 1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening whwere they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910. On 13 March 2015, and at times prior thereto, electrical disconnect switches had materials stored in front of them blocking the quick and ready access to the panels in the event of an emergency, exposing employees to a tripping hazard. Instance A) Pipes, piping rack, and other stands were blocking the disconnect switch for the vertical saw within Bay 1. Instance B) A pedestal floor fan and a metal table were blocking the disconnect switch for an arc welder within Bay 3.

On 13 March 2015, and at times prior thereto, a pull box was missing the cover allowing the energized wires within to be unguarded near the floor level and below a four-plug outlet, exposing the employees utilizing the outlet to an electrical hazard of approximately 120 volts.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws. On 13 March 2015, and at times prior thereto, within Bay 1, a three plug extension cord tree had strain relief damage at the female plug end, exposing employees using the cord to an electrical hazard of approximately 120 volts.
