29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: (a) Facility- 355 Washington Road, Washington, PA: On or about March 12, 2015 the company built platforms for the order selectors without the manufacturers prior written approval and failed to mark the additions on the powered industrial trucks to identify the attachments changes/additions to the powered industrial trucks.
