29 CFR 1926.1433(d)(8): Belts, gears, shafts, pulleys, sprockets, spindles, drums, fly wheels, chains, and other parts or components that reciprocate, rotate or otherwise move were not guarded where contact by employees (except for maintenance and repair employees) is possible in the performance of normal duties. a) Worksite: On or about March 9, 2015 (while engaged in the assembly of a crane) employees re-spooling the main running cable on an American crawler crane model number 9270, were exposed to unguarded moving parts. An employee's loose safety harness lanyard caught in the shaft/flywheel of the power-down shaft, pulling him into the shaft.
