29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. (a) 1000 South 4th Street, Hamburg Pennsylvania - Employees operate powered industrial trucks to lift and move recycled products. The employer did not ensure that each employer received formal instruction prior to operating powered industrial trucks, on or about February 27, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA: (a) 100 South 4th Street, Hamburg Pennsylvania - An employee suffered a hospitalization on February 27, 2015 as a result of a broken ankle from a Toyota sit down forklift tipping over. The employer failed to report this work-related hospitalization to OSHA within 24 hours. (The employer reported this hospitalization to OSHA on March 6, 2015.) No abatement certification or documentation required.
