29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Press 215: On or about March 10, 2015 and at times prior thereto, the employer did not ensure its written lockout tagout procedure included the isolation and control of all energy sources (e.g. pneumatic and hydraulic) which exposed maintenance employees to injuries related to exposures to those energy sources and moving machinery. b) Press 215: On or about March 10, 2015 and at times prior thereto, the employer did not ensure lockout tagout procedures were utilized when machine guarding was bypassed to clean roller plates which exposed the press operators to injuries related to moving machinery.
