29 CFR 1910.146(e)(1): Before entry was authorized, the employer did not document the completion of measures required by 29 CFR 1910.146(d)(3) by preparing an entry permit:  (a) New Castle City water meter pit, Johnson Way & Centerpoint Blvd, New Castle, DE - The employer did not prepare entry permits for multiple entries into this confined space in order to ensure safe permit space entry operations for employees exposed to potential hazards such as asphyxiation and fall hazards.  On or about March 12, 2015 and dates prior.  Pursuant to 29 CFR 1903.19, the employer must submit abatement certification, required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

On or about March 12, 2015 and dates prior.  Pursuant to 29 CFR 1903.19, the employer must submit abatement certification, required within 10 days after abatement date.
