29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:    a) T-4-31 - Employees installing a metal roof were exposed to a fall hazard of approximately seventeen (17) feet from an aerial lift without the use of at least a body belt and lanyard , on or about March 16th, 2015.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    a) T-4-19 - Employees installing metal roofing on an approximate 3X12 pitch roof were exposed to a fall hazard of approximately six (6) foot six (6) inches while not protected by the use of a guardrail system, safety net system or a personal fall arrest system, on or about March 16th, 2015.
