29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, and rotating parts:   Plant, Deboning Cone Line #5 - On or about 03/16/15 and at times prior thereto the horizontal conveyor in the wing segmenter (Prime Equipment Group Model CWS-1, S.N.  SN2529 ) was not guarded on the sides to prevent accidental contact with pinch points created by the vertical steel pins (displacing in the horizontal direction) and the stationary structure of the machine. Employees were exposed to amputation hazards.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.147(c)(6)(ii): The periodic inspection certification of energy control procedures did not contain the required documentation on identification of the machine or equipment, inspection date, employees included, and the person performing the inspection.  a) Plant - On or about 03/16/15 and at times prior thereto employees conducted repairs/maintenace of machinery including (but not limited to) the equipment in the cone lines #1 to #5, and the equipment listed in the document equipment inventory which is subjected to lockout/tagout.  The employer did not certify that periodic and at least annual inspections of the individual written lockout/tagout procedures had been completed and maintained up to date.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
