(b) Worksite, south side of home - Two employees installing cedar shake shingle siding with Bostitch pneumatic nail guns, Model # N66C-1 / Serial # 14062140H BK and Model # N66C / Serial # 165355, were not provided with; or using any form of eye protection on, or about March 16, 2015.  (c) Worksite, north side of home - An employee cross cutting cedar shake shingle siding with the Skilsaw table saw, Model # 3310 / Serial # 201, was not provided with; or using any form of eye protection on, or about March 16, 2015.   Note: In addition to abatement certification, the employer is required to submit abatement documentation for this violation in accordance with 29 CFR 1903.19.

29 CFR 1926.304(h)(1): Each circular crosscut table saw was not guarded by a hood which completely enclosed that portion of the saw blade above the table and that portion of the saw above the material being cut:   (a) Worksite, north side of home - An employee cross cutting cedar shake siding was operating the Skilsaw table saw, Model # 3310 / Serial # 201, which was not equipped with a hood guard that enclosed the saw blade above the table; and the material being cut on, or about March 16, 2015.   Note 1: In addition to abatement certification, the employer is required to submit abatement documentation for this violation, in accordance with 29 CFR 1903.19.    Note 2:  See paragraph (i)(1) of this section for additional requirements for hand-fed crosscut table saws.

The scaffold was not inspected by a competent person before the work shift on, or about March 16, 2015.
