29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: Web press, 750 Crawford Street, Fitchburg, MA: On or about March 16, 2015: electrical junction boxes and switches were observed on the web presses which were not provided with a cover or were not securely fastened exposing employees to electrical shocks and burns. (a) Web press, junction box, missing cover. (b) Web press, water pump switch, unsecured cover plate. (c) Web press, junction box, near gear guard, cover plate not fully secured.
