29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed: 104 Lamartine St, Worcester MA On March 17, 2015 and at times prior, two chains and sprocket wheels that transmitted power to two conveyor belt machines were not enclosed by guarding and were not more than 7 feet above the floor. On March 13, 2015, an employee came in contact with a chain and sprocket wheel mechanism resulting in a partial arm amputation injury.
