29 CFR 1910.146(k)(2)(iv): The employer did not ensure that affected employees practice making permit space rescues at least once every 12 months, by means of simulated rescue operations in which they remove dummies, manikins, or actual persons from the actual permit spaces or from representative permit spaces: a) At the facility, on or about March 17, 2015 - Affected employees have not practiced a rescue from a vacuum truck tank by performing a tank breach every 12 months. Employees enter the tank to weld components.
