29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  a- North Side Of Building - 1137 Bordentown Road, Burlington NJ: On or about 03/16/15, company employees performing stucco application and other related activities from a prefabricated, tubular, welded framed scaffold were exposed to falls in excess of 16 feet to the ground surface below when the scaffold they were working from was not fully planked.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. 29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person.  a- North Side Of Building - 1137 Bordentown Road, Burlington NJ: On or about 03/16/15, company employees performing stucco application and other related activities from a prefabricated, tubular, welded framed scaffold were exposed to falls in excess of 16 feet to the ground surface below when the scaffold they were working from was not fully planked, was missing mid/top rails and end stops, was set up so employees were working next to a live power line, etc.  The scaffolding was not erected by a competent person.  b- West Side Of Building - 1137 Bordentown Road, Burlington NJ: On or about 03/16/15 a company employee performing stucco application and other related activities from a single metal pick set between two frames of a prefabricated, tubular welded framed scaffold was exposed to falls in excess of 13 feet to the ground surface below, when the pick was not supplied with mid/top rails.  The scaffolding was not erected by a competent person.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
