29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    On or about February 23, 2014 bread divider operators were not required to deenergize and lock out the power source for the bread divider prior to lifting the guard and cleaning dough and oil form the machine.  This condition exposed employees to the hazard of the unexpected energization of the equipment resulting in contact with moving parts such as the ram and knife.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:       On or about March 17, 2015 and prior thereto the ingoing nip points on conveyor belts were not guarded exposing employees to the hazard of being caught between the belt and the rotating drum in the following instances:   a.    Takeaway belt at the front of the bread divider   b.    Belt from the bread divider to the rounder   c.    Bottom of the bun dough elevator belts   d.    Bottom of the bread dough elevator belts       On or about March 17, 2015 and prior thereto, the guard to the rounder had openings which exposed employees to the hazard of contacting the rotating rounder in the following instances:   e.    Opening around the conveyor belt entering the rounder from the bread divider   f.    Opening around the chute exiting the rounder       On or about March 19, 2015 the rotating augers at the bottom of the hoppers at the dough pumps were not equipped with guards exposing employees to the hazard of being caught between the two augers or between the hopper and augers in the following instances:   g.    The trip wire around the top of the bun dough pump hopper was installed with slack so that the wire could rest on the rim of the hopper and not stop movement of the augers     h.     The trip wire around the top of the bread dough pump hopper was not installed  in such a manner as to prevent movement of the augers specifically due to deflection of the outside side of the hopper       On or about March 17, 2015 the ram  in the bread divider was not guarded, exposing employees to the hazard of contacting the moving ram or being caught between the ram and a stationary part in the following instance:   i.        Scraping the hopper above the bread divider

29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting:    a.    On or about March 17, 2015 the rotating shaft on the takeaway belt located under the front of the bread divider that the dough drops on was not guarded.  Employees were exposed to the hazard of being caught by the rotating shaft.        b.    On or about March 17, 2015 the rotating shaft on the conveyor from the takeaway belt to the rounder was not guarded.   Employees were exposed to the hazard of being caught by the rotating shaft.        c.    On or about March 17, 2015 the rotating shaft on the dough belt that dropped dough into the hopper at the top of the bread divider was not guarded.  Employees were exposed to the hazard of being caught by the rotating shaft.      d.  On or about March 19, 2015 the rotating shaft on the motor drive to the augers was not guarded.   Employees were exposed to the hazard of being caught by the rotating shaft.

29 CFR 1910.263(f)(3): Rear cover(s) of the divider(s) were not provided with a limit switch to prevent machine operation while the cover(s) were open:       On or about February 23, 2015, the interlocked switch on the guard of the bread divider did not function as intended allowing the equipment to operate with the guard open.  This condition exposed employees to the hazard of contacting the knife and moving parts of the bread divider.
