29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments: Worksite located at 4400 S. 96th Street, Omaha, NE: The employer failed to ensure employee(s) were trained in and familiar with electrical safety related work practices. On or about March 13, 2015, the employer failed to ensure that employees working on live circuits and/or conductors either completely de-energized the circuits and/or implemented electrical safe work practices such as but not limited to the use of PPE, shielding or other work practices. This failure resulted in an injury to an employee when one of the live conductors energized the conduit shocking the employee while working on a rooftop cooling unit.

29 CFR 1910.333(b)(2)(i): The employer failed to maintain a written copy of the procedures outlined in paragraph (b)(2): Repack Area of Omaha Steaks International located at 4400 S 96th Street, Omaha, NE: The employer failed to document the procedure to verify that energized electrical circuit parts or equipment were de-energized prior to permitting work to proceed. On or about March 13, 2015, a maintenance employee was injured while pulling electrical wire from a conduit which contained other energized conductors. The employer allowed maintenance employees to perform this work without requiring the live conductors located in the same conduit to be de-energized prior to starting work.

29CFR 1910.333(c)(2): Persons who were not qualified worked on electrical circuit parts or equipment that had not been de-energized under the procedures of paragraph (b) of this section. Such persons were not capable of working safely on energized circuits and were not familiar with the proper use of special precautionary techniques, personal protective equipment, insulating and shielding materials and insulated tools. On or about March 13, 2015, a maintenance employee was injured while pulling electrical wire from a conduit.
