Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact. a) On or about March 11, 2015 employee was not protected from fall hazards that exceed 6' on PSM #4 while traversing point to point on the elevated mail slides without personal protective fall protection equipment being used/utilized exposing employee to fall hazards. b) Employees were not protected from fall hazards that exceed 6' on PSM #4 while traversing point to point on the elevated mail slides without personal protective fall protection equipment being used/utilized exposing employees to fall hazards. c) Employees were not protected from fall hazards that exceed 6' on PSM #3 while traversing point to point on the elevated mail slides without personal protective fall protection equipment being used/utilized exposing employees to fall hazards.

All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). a) On or about March 11, 2015 the electrical disconnect on the Mail Slide PSM#4 was not shut off to the conveyer and pusher plate while removing jams, exposing employee pinch point and/or moving parts. b) The electrical disconnect for Mail Slide PSM #4 was not shut off to the conveyer and pusher plate while removing jams, exposing employees to pinch point and/or moving parts. c) The electrical disconnect for Mail Slide PSM #3 was not shut off to the conveyer and pusher plate while removing jams, exposing employees to pinch point and/or moving parts.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a) On or about March 11, 2015 machine guarding was not provided over the Pusher Plate Deflector roller attachment points on PSM#4, in that while removing mail from the slide employees hand entered the pinch points. b) Machine guarding was not provided over the Pusher Plate Deflector roller attachment points on PSM#3, exposing employees to pinch points hazards while removing mail from the slide.
