29 CFR 1910.212(a)(4): Barrels, containers, and drums. Revolving drums, barrels, and containers shall be guarded by an enclosure which is interlocked with the drive mechanism, so that the barrel, drum, or container cannot revolve unless the guard enclosure is in place. Operations / Production facility - On or about March 18, 2015 - Employees were required to operate the Niederberger sander / grinder on a daily basis. The interlocks on the grinder did not effectively ensure that all operations (roller drum) stopped when engaged. Employees were thereby exposed to the hazards associated with rotating / moving parts.

29 CFR 1910.147(c)(7)(iv): The employer shall certify that employee training has been accomplished and is being kept up to date. The certification shall contain each employee's name and dates of training. Paperwork deficiency - On or about March 18, 2015 - Employees were required to work on and around machinery subject to the requirements of lockout / tagout and or otherwise covered by OSHA standard 1910.147. The employer failed to certify the training provided to employees as discussed / required in this standard.
