Site: 21 East 13th Street New York, NY On or about 3/16/15 a) The exit door in the rear of the establishment did not lead to directly toward the outside of the establishment. The exit lead to a yard which is full of debris and bricks.

The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp were not provided where the exit route was not substantially level. Site: 21 East 13th Street New York. NY On or about3/16/15 a) The rear emergency exit door was not maintained free and unobstructed from materials. The emergency exit door was blocked by chairs, a hand bag and garbage cans.
