(b)  On or about 03/17/2015 - Main plant, the Baldor floor grinder (grey) located by the lathes was adjusted to 1/2 inch from the wheel on the left side and 2 inches  from the wheel on the right side.

29 CFR 1910.215(d)(3): The contact surface(s) of wheel(s), blotter(s) or flange(s) on grinding machine(s) were not flat and free or foreign matter:  (a)  On or about 03/17/2015 - Main plant, the stones on the Baldor floor grinder (green) located by the stairs leading to the upper level the stone were not flush and free of debris. The stone had groves where drill bits had been sharpened on the right side and the stone on the left side was worn down.  (b)  On or about 03/17/2015 - Main plant, the stones on the Baldor floor grinder (grey) located by the lathes were not flush and free of debris.  The stone on the right side was beveled on one side where tools had been sharpened and the stone on the right side was also not flush.
