29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     JOB SITE: 557 Mapleview Drive, University City, MO 63130, on or about 3/18/2015, at this work site, workers were observed on top of an 6:12 pitch roof, approximately 12 feet eave to ground level, actively removing old roof shingles, and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures.  The workers were exposed to a fall hazard.    The EDUARDO ORTEGA dba EDUARDO ORTEGA was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 314737651, citation number 1, item number 3, and was affirmed as a final order on 9/16/2010, with respect to a workplace located at 345 Meadowbrooke Drive, Ballwin, MO 63011.
