29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury:  At the maintenance shop located at 8123 Christensen Ln, Omaha, NE 68122, the employer is failing to protect employees servicing the hydraulics systems on the McNeilus rear load, front load and side load garbage trucks from struck-by and caught-in hazards in that; a written energy control program was not established to clearly document and communicate proper energy control and lockout/tagout procedures to be followed while servicing garbage trucks, initial and periodic training of authorized and affected employees, and the completion of periodic inspections at least annually to ensure effective implementation of the energy control program.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  At the workplace located at 8123 Christiansen Ln, Omaha, NE 68122 the employer is failing to protect employees from chemical hazards such as burns and skin irritation while using chemicals including but not limited to TerraCair DEF that contains anhydrous ammonia in that; a hazard communication program was not developed and maintained that describes how training on the safe use and handling of chemicals is to be conducted, how to ensure that product labeling communicating inherent chemical hazards to employees is in place, and how safety data sheets are to be maintained so that they are regularly updated and accessible to the employees in the workplace.
