29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered. Location - 3 Freedom Circle, Portsmouth, NH: On or about February 16, 2015, employees were working on the roof of a two story building shoveling snow. The pitch of the roof was 5 in 12. None of the employees on the roof were using fall protection.
