The employer did not ensure a Maintenance employee was removed from the Buffoli 526's hydraulic chiller while the chiller's fan was energized.

The employer did not request the forklift manufacturer's prior written approval before the fork extensions, which affected the capacity and safe operation of the Standup Forklift, were used.

The employer did not obtain or mark the Standup Forklift with the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered when front-end attachments other than factory installed attachments were used.
