29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) For employees working at 3200 Manatee Avenue West in Bradenton as observed on or about 18 March, 2015. Employees were installing roofing tile on a 5 1/2:12 pitch commercial roof above 6 feet without the use of a fall protection system in place. Employees were exposed to a fall height of approximately 11 feet 3 inches.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) For employees working at 3200 Manatee Avenue West in Bradenton as observed on or about 18 March, 2015. Employees were using an extension ladder to access the upper work surface that was only extended approximately 2 feet above the upper landing surface. Employees were exposed to a fall height of approximately 11 feet 3 inches.
