29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  (a)  2601 River Road, Conestoga Pennsylvania, ploy tray - Employees place body parts in the poly tray to clear jams and clean after an alarm sounds and maintenance conducts inspections.  Prior to unjamming and cleaning the employee, does not turn off the machine [per 1910.147(d)(2) requirements].  As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], lockout/tagout device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)] did not get implemented to protect the employee from machine servicing hazards, including, but not limited to caught in and struck by  , on or about March 19. 2015.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
