29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:     a.  An employee installing exterior insulation was exposed to a fall hazard of approximately 40 feet while working out of an aerial lift and the employer did not ensure the employee attached the lanyard to the anchorage point in the basket.    b. An employee painting the outside structure was exposed to a fall hazard of approximately 40 feet while working out of an aerial lift and the employer did not ensure the employee attached the lanyard to the anchorage point in the basket.
