29 CFR 1904.30(a): Basic requirement. You must keep a separate OSHA 300 Log for each establishment that is expected to be in operation for one year or longer. On or about March 19, 2015, Pilot Thomas Logistics did not have separate logs of all Work-Related Injuries and Illnesses (OSHA Form 300), and/or the Summary of Work-Related Injuries and Illnesses (OSHA form 300-A) broken down by each separate establishment nationwide. The injuries and illnesses could not be broken down by individual establishment for review of trends. On November 2, 2014, an employee received a laceration to the cheek from a hose at the Louisville Ohio Pilot Thomas Logistics location.
