On or about March 19, 2015, Pilot Thomas Logistics did not have separate logs of all  Work-Related Injuries and Illnesses (OSHA Form 300), and/or the Summary of Work-Related Injuries and Illnesses (OSHA form 300-A) broken down by each separate establishment nationwide. The injuries and illnesses could not be broken down by individual establishment for review of trends.  On November 2, 2014, an employee received a laceration to the cheek from a hose at the Louisville Ohio Pilot Thomas Logistics location.
