The power cord to a DeWalt 7 1/4 inch portable circular saw, S/N 116471, used for cutting boards to length, had been spliced and the tape applied to the outer insulation was not equivalent to the original insulaiton, as observed on March 19, 2015. 29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): (a) Green Chain Area. Prior to use, no visual inspection was made of the DeWalt 7 1/4 inch portable circular saw, S/N 116471, to identify hazards such as the spliced power cord that had been covered with tape that was not equivalent to the original insulation of the cord, as observed on March 19, 2015. (c) The 2015 OSHA 300 log (column K) did not list the number of days away from work for an injury that occurred on March 12, 2015, as determined on March 19, 2015. (d) The 2015 OSHA 300 log (column I) listed a January 16 injury as a job transfer or restriction, but 4 days away from work were listed in column K, as determined on March 19, 2015.
