The back up alarm was not operable on the Taylor Eagle forklift, Model TEB-220M, S/N S C2-21893, as observed on March 19, 2015.    (b)    Yard Area.  The Taylor Eagle forklift, had broken glass on the left side of the cab and the windshield was cracked, as observed on March 19, 2015.    ABATEMENT CERTIFICATION REQUIRED FOR THIS ITEM

29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service and an employee was using it before repairs and tests necessary to render the equipment safe were made:    (a)    Green Chain Area.  The power cord to a DeWalt 7 1/4 inch portable circular saw, S/N 116471, used for cutting boards to length, had been spliced and the tape applied to the outer insulation was not equivalent to the original insulaiton, as observed on March 19, 2015. 29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  (a)    Green Chain Area.  Prior to use, no visual inspection was made of the DeWalt 7 1/4 inch portable circular saw, S/N 116471, to identify hazards such as the spliced power cord that had been covered with tape that was not equivalent to the original insulation of the cord, as observed on March 19, 2015. 29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in the detail as required by the regulation:    (a)    The 2015 OSHA 300 log of Work-Related Injuries and Illnesses had entries for three employees employed by Mingo Wood Products but the Establishment Name on the form was listed as Cabin Creek Lumber, as determined on March 19, 2015.    (b)    The 2015 OSHA 300 log (column F) did not describe the injury and object that directly injured the three injuries listed on the form, as determined on March 19, 2015.    (c)    The 2015 OSHA 300 log (column K) did not list the number of days away from work for an injury that occurred on March 12, 2015, as determined on March 19, 2015.    (d)    The 2015 OSHA 300 log (column I) listed a January 16 injury as a job transfer or restriction, but 4 days away from work were listed in column K, as determined on March 19, 2015.
