29 CFR 1910.32(a): Protective equipment was not provided when necessary whenever hazards capable of causing injury and impairment were encountered: a. Clark Order Picker Truck. The employer provided personal protective equipment that did not adequately protect employees from fall hazards when stepping off of the order picker truck platform to place or retrieve merchandise at heights ranging from approximately 5-12 feet above ground. Employees wore a body belt with a lanyard that was too long to provide protection in the event of a fall. There is no abatement documentation or certification required for this item.

An extension cord (110-120v) was used in lieu of fixed conduit to provide power to two television screens. The cord was run from the equipment up across the ceiling and back down along a structural support column to an electrical outlet, exposing employees to potential fire hazards.
