29 CFR 1910.95(b)(1): When employees are subjected to sound exceeding those listed in Table G-16, feasible administrative or engineering controls shall be utilized. If such controls fail to reduce sound levels within the levels of Table G-16, personal protective equipment shall be provided and used to reduce sound levels within the levels of the table.      a)  In the welding shop area, an employee working as a welder was exposed to noise levels of 112.1% of OSHA permissible exposure level. 112.1% is equivalent to 90.8 dBA (8-hour time weighted average). This noise survey was conducted during a normal work shift on the welder for a period 370 minutes. Zero exposure was assumed for the unsampled period of 110 minutes.   Employer did not have a monitoring program.      b)  In the welding shop area, an employee working as a welder was exposed to noise levels of 113.4% of OSHA permissible exposure level.  113.4% is equivalent to 90.9 dBA (8-hour time weighted average).  This noise survey was conducted during a normal work shift on the welder for a period 421 minutes.  Zero exposure was assumed for the unsampled period of 59 minutes.   Employer did not have a monitoring program.      c)  In the shop area, an employee working as a blaster was exposed to noise levels of 136.1% of OSHA permissible exposure level. 136.1% is equivalent to 92.2 dBA (8-hour time weighted average).  This noise survey was conducted during a normal work shift on the blaster for a period 298 minutes. Zero exposure was assumed for the unsampled period of 182 minutes.  Employer did not have a monitoring program.

29 CFR 1910.134(c)(2)(i): An employer may provide respirators at the request of employees or permit employees to use their own respirators, if the employer determines that such respirator use will not in itself create a hazard. If the employer determines that any voluntary respirator use is permissible, the employer did not provide the respirator users with the information contained in Appendix D to this section ("Information for Employees Using Respirators When Not Required Under the Standard"); and       At the welding shop area; the employer did not provide appendix D to  employees who wore a 3M particulate dust mask, model 8512 while gauging and welding on carbon steel, exposing employees to inhalation hazards.
