29 CFR 1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face: location: Grand Concourse Service Rd., west side between 171 St. & 172 St. on or about: 3/16/15 a) Employees were working in a 13 feet deep trench that was inadequately protected from loose rock. A rock dislodged from the excavation face.
