The employer does not ensure that the side rails on portable ladders used for access to an upper landing surface were extended at least 3 feet above the upper landing surface being accessed.  This violation was observed on or about March 19, 2015, at a residential home construction site where employees were exposed to a fall hazard when accessing the roof with a ladder whose side rails did not extend at least 3 feet above the landing surface.
