29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). The employer does not ensure that each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were protected by guardrail systems, safety net system, or personal fall arrest system. This violation was observed on or about March 19, 2015 when employees were exposed to a fall hazard greater than 6 feet when performing roofing work without a fall protection system. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels are protected by guardrail systems, safety net system, or personal fall arrest system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. The employer does not ensure that the side rails on portable ladders used for access to an upper landing surface were extended at least 3 feet above the upper landing surface being accessed. This violation was observed on or about March 19, 2015, at a residential home construction site where employees were exposed to a fall hazard when accessing the roof with a ladder whose side rails did not extend at least 3 feet above the landing surface. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the side rails on portable ladders are extended at least 3 feet above the upper landing surface being accessed.
