29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:    This violation occurred on or about March 18, 2015 when employees were exposed to fall hazards while using a stepladder to access the roof of a single family residential home.  The point of access contained a break in elevation of approximately 4 feet between the top of the ladder and the roof of the residential building.
