29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a. On or about 02/23/2015 at the above addressed jobsite, employees were observed installing roofing material on a steep slope roof, while wearing a Personal Fall Arrest System (PFAS) harness, but without the lifeline/lanyard connected to the harness, therefore exposing the employees to a fall hazard.

29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used: a. On or about 02/23/2015 at the above addressed jobsite, employees were using a Personal Fall Arrest System (PFAS) while installing roofing material, for which the employer had not provided the employees with proper training on the care, use, and inspection/maintenance of the conventional (PFAS) fall protection system, therefore exposing the employees to fall hazards.
