29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a)Band-It-IDEX, Inc., at 4799 Dahlia St., Denver, CO 80216: On and before March 2nd, 2015, the employer did not ensure that procedures were developed, documented and utilized for the control of, but not limited to, unintentional re-energization of pneumatic energy when employees were required to place any part of his or her body into the point of operation. This condition exposed the employees to amputation hazards during troubleshooting activities on the Automated Clamp Winding machines.

This condition exposed employees to the hazards of inadequate lockout procedures for the Automated Clamp Winding machines during troubleshooting activities.

This condition exposed the employees to struck by and amputation type injuries caused by rotating parts and points of operation associated with ACW machines.
