29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment would be isolated from the energy source and rendered inoperative:  a) On March 19, 2015 and at times prior thereto the employer relied on informal procedures for the control of potentially hazardous energy when employees performed scheduled and un-scheduled maintenance on the Amada laser punch, and other machines and equipment through-out the facility.  Informal training did not ensure that authorized employees were provided with necessary information about the identification and location of energy sources, process of installing, removing, transferring and testing the lockout / tag-out devices. 29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Welding area: On March 19, 2015 and at times prior thereto the Grizzly combination sander, model H6070, was missing the belt safety cover.    b) Welding area: On March 19, 2015 and at times prior thereto the Grizzly combination sander, model H6070, was missing the sanding belt table that supports the work piece at various angles for belt sanding.   c) Welding area: On March 19, 2015 and at times prior thereto the Grizzly combination sander, model H6070, was missing a cover / guard over the spindle and nut where the abrasive disc had been removed.    d) Production area: On March 19, 2015 and at times prior thereto, an employee operating the 35 ton Amada press brake, worked with small work pieces that did not allow his fingers to be kept a safe distance away from the point of operation.
