29 CFR 1910.107(b)(5)(i): Visible gauges, audible alarms, or pressure activated devices were not installed to indicate or insure that the required air velocity was maintained:    a) Standard Tools & Equipment Co. spray booth    The  Dwyer monometer mounted on the STE (Standard Tools & Equipment) spray booth for truck painting was inoperative during spray painting operations involving the use of flammable solvents, to include but not limited to, heptane, toluene and methyl ethyl ketone.    Condition observed on or about 5/4/2015.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    (a) Truck Body East, 50 Main St., Orange, NJ    The employer failed to develop and implement a written respiratory protection program for employees exposed to organic solvents and hexamethylene diisocyanate and required to wear a 3M air line respirator while spray painting; and for employees who voluntarily wear 3M negative pressure half face respirators during sanding or maintenance operations.
