29 CFR 1910.37(a)(3): Exit routes were not continuously maintained free of obstructions or impediments to full instant use in the case of fire or other emergency: a/ GMA Accessories/Edison NJ Site: Warehouse. 1/ Receiving Area. Emergency exit door was blocked by a large plastic garbage container, a wooden pallet, and other bulk storage material. 2/ Packaging/Inspection Area: Emergency exit door was blocked by stacks of wooden pallets. Emergency exit doors were blocked preventing free escape from the facility in the event of a fire or other emergency. Observed on/or about 03/19/2015.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment: a/ GMA Accessories, Inc. DBA/Capelli New York/Edison NJ Site: Warehouse. The hazard assessment to identify potential hazards permanent and temporary employees would be exposed to was not documented and certified to ensure that required personal protective equipment had been selected and made available where needed. Employees are exposed to hazards associated with (including but not limited to) handling packages/boxes, operating powered industrial trucks, using manual box cutting tools, checking powered industrial truck battery fluid levels, and performing sorting operations. Observed on/or about 03/19/2015.
