29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket and/or used planks, ladders, or other devices for a work position: a) 433 West St., Amherst, MA: On or about March 17, 2015, an employee was working from a platform attached to a raised aerial lift which was outside of the integrated basket/guardrails of that aerial lift more than 20 feet above the ground below. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: 433 West St., Amherst, MA: On or about March 17, 2015, employees were working from; a) a platform attached to a raised aerial lift which was outside of the integrated basket/guardrails of that aerial lift and b) an open sided terrace both of which were more than 20 feet above the ground below without fall protection. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
