29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition when it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact: a. On or about February 26, 2015, at the above addressed jobsite, an employee was not provided with appropriate personal protective equipment to protect extremities while removing glass windows.

29 CFR 1926.59: An employer engaged in construction activities did not comply with the requirements set forth in Part 1910, Section 1200, Hazard Communication: a. On or about March 5, 2015, at the above addressed jobsite, employees who worked with a hazardous chemical (i.e. Dow Corning contractor's concrete sealant gray) were not provided with information and training, including the required list of hazardous chemicals and safety data sheets of a hazard communication program.
