29 CFR 1910.67(c)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. (a) Front yard - On or about March 19, 2015 an employee was exposed to a fall hazard while trimming trees in an International Durastar Bucket Truck. Employee was elevated 60 feet above the ground without body harness or body belt and lanyard.

29 CFR 1910.135(a)(1): The employer shall ensure that each affected employee wears a protective helmet when working in areas where there is a potential for injury to the head from falling objects. (a) Front Yard - On or about March 19, 2015 employees were exposed to a struck-by hazard while trimming trees in an International Durastar bucket truck. One employee was in the bucket while other employees were working on the ground 60 feet below bucket. Employees were not wearing hardhats.
