29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms were not kept clean and orderly and in a sanitary condition.    On or about 20 March 2015, the employer did not ensure that thorough housekeeping was conducted within the shop area resulting in extensive dust accumulation on the window ledges, equipment and electrical sockets, exposing employees to hazards such as but not limited to combustible dust fires.

29 CFR 1910.134(f)(2): The employer did not ensure that employees using tight-fitting facepiece respirators were fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) was used, and at least annually thereafter.    On or about 20 March 2015, the employer did not provide fit testing for employees working in the shop, who were using 3M N95 particulate respirators while sanding, and 3M half face respirator while laminating.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about 20 March 2015, there were two(2) instances in which the employer did not provide machine guarding for employee protection:   a) The rotating chuck on the Ridgid drill press was not guarded, exposing employees to hazards such as those created by being caught in between the point of operation and the material.  b) The blade on the band saw (machine no 5) was not guarded exposing employees to amputation hazards.

29 CFR 1910.334(a)(2)(i): Portable cord- and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence for possible internal damage (such as pinched or  crushed outer jacket):    On or about 20 March 2015, in the countertop area, there were two (2) instances where the employer did not visually inspect and repair damaged electrical cords being used to power equipment, exposing employees to electrical hazards:   Instance a) An electrical cord that had damage to the outer sheath jacket, exposing the internal insulated conductors, was used to power a Dewalt miter saw.  Instance b)  A yellow extension cord that had damage to the outer sheath jacket, exposing the internal insulated conductors, was used as a power tap.
