(a) On or about March, 5, 2015 at worksite 11325 NE 83rd Street, Kansas City, Missouri--employees were exposed to serious eye injury while using a Hibachi brand pneumatic nail gun without eye protection. 29 CFR 1926.501(b)(13):   Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) On or about March, 5, 2015 at worksite 11315 NE 83rd Street, Kansas City, Missouri- Employees were performing roofing operations at an elevated height of approximately 21-23 feet, without any means of fall protection by way of guardrail systems, safety net systems, or personal fall arrest systems.
