29 CFR 1910.137(b)(2)(viii): Electrical protective equipment was not subjected to periodic electrical tests. Test voltages and the maximum intervals between tests shall be in accordance with Table I-5 and Table I-6. On or about March 18, 2015 employees acting in the capacity of electricians were exposed to electrical hazards where required to perform work on energized electrical equipment using "Salisbury" rubber insulating gloves which had not been subjected to testing every six months.

29 CFR 1910.95(d)(1): When information indicates that any employee's exposure may equal or exceed an 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program. On or about March 18, 2015 employees were potentially exposed to noise hazards where electrical employees may be exposed to sound levels exceeding an eight-hour, time weighted average of 85 decibels during quarterly and annual maintenance activities in the "Hitec" engine room. The employer has not performed initial monitoring to identify employees for inclusion in an effective hearing conservation program compliant with all requirements of 29 CFR 1910.95.
