29 CFR 1926.100(a):   Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   A) Exterior front on a ladder: - Employees were exposed to head injuries while working below framing activities. The employees were not protected by hard hats or protective helmets, on or about January 29th, 2015.  B) Upper level of structure on a ladder: An employee was exposed to head injuries while working below framing activities. The employee was not protected by a hard hat or protective helmet, on or about January 29th, 2015.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:                                                                                                             A) Upper level of structure: Employees were exposed to eye/face injuries while using pneumatic nail guns and electric saws and were not protected with any type of eye/face protection, on or about January 29th, 2015.   B) Lower level on ladder: An employee was exposed to eye/face injuries while using a pneumatic nail gun and was not protected with any type of eye/face protection, on or about January 29th, 2015.
